Date: 06.02.2009.
1) Registration forms (subject to vacancies in various
classes) would be available in the Vidyalaya Office on payment of Rs.100/- on
all working days between
2) Registration forms, completed in all respects and
accompanied by all required documents must be submitted to the Vidyalaya office
within the prescribed date i.e. up to 20.03.2009 in between

copies of the following documents
would be required to be submitted along with the application form for
registration.
i)
(A) For class I: - Certificate of proof
of age in the form of birth certificate issued by the authority competent to
register birth. This will include certificate from Notified Area Council/
Municipality/ Municipal Corporation extract about the date of birth from
records of Village Panchayat,
(B)For other classes, recorded in the
Transfer Certificate issued by a school recognized by the State Education
Department. A certificate of Date of Birth from the school studying would be
attached in the form.
ii)
Certificate that
the child belongs to the SC. /ST/SEBC/Minority Community wherever applicable
issued by the Competent Authority not below the rank of Tahsildar. 17% seats
reserved for SC candidates and 22 % seats for reserved for ST candidates. No
reservation for SEBC candidates.
iii)
A certificate of
Physically Handicapped issued by Competent Authority wherever applicable.
iv)
A service
certificate showing the number of transfers during the preceding 7 years i.e.,
from 01.04.2002 to 31.03.2009
duly signed and stamped by the Head of office bearing the name and
designation and other relevant particulars in capital letter.
4) Recent passport size photograph need
to be fixed on the Application form.
5) For non- government officials a copy of the proof of
residence (such as Ration Card, Electric bill, Driving License, Voter ID Card
etc. which should be in the Parent’s name).
6) Parents having Single Girl child may apply along with
a copy of an affidavit from First Class Magistrate stating that they have single
girl child and that there is no other sibling. (Applicable for admission to
Class-I and Classes-VI onwards).
7) Mere registration will not confer a right to
admission.
8) Incomplete application forms shall
normally be rejected.
9) The Principal shall have the right to cancel admission
secured on the basis of any wrong certificate/ information forthwith and no
appeal against such action of the Principal shall be entertained.
10) The admission test details (Class II onwards) if vacancy exists, will be displayed in
the Vidyalaya notice board in the 2nd week of March.

PRINCIPAL
NOTE: - A child must be at least 5 years old on 31st
of March of the calendar year in which admission is sought for class-I and
class-II. For remaining classes, the eligible age will be calculated as on
30th September of the academic year.
From 2010-11 the qualifying age will be applicable up to Class-III and
so on progressively year after year.

Sir/Madam,
I
would like to inform you that the Registration Forms for admission for the year
2009-10 for Pre- Primary & classes I to XII(science) (Subject to Vacancies)
are available on payment of Rs.100/- on any working days between 11:00 a.m. to
12:00 p.m. from 15.02.2009 to 20.03.2009. Last date for submission of
Registration From is 20.03.2009.
A
child must be at least 4 year for Pre-Primary, 5 years old on 31st of March of
the calendar year in which admission is sought for class-I and Class-II. For
remaining classes, the eligible age will be calculated as on 30th September of
the academic year.
Registration
for 03 sections of class XI (Science) shall be taken up immediately after the
declaration of the result of class X.
17%
& 22% seats are reserved for SC & ST candidates. Seats are also
reserved for the Physically Handicapped as per admission guidelines issued by
KVS.
Parents
having single girl child may apply along with a copy of an affidavit stating that
they have single girl child and that there is no other sibling. (Applicable for admission to Class-I and
Classes-VI onwards)

i)
The child has been in the same course of studies i.e.
in CBSE – affiliated school.
ii)
The child must have obtained not less than 50% marks in
class IX / XI(SCIENCE) examination.
iii)
The child should OTHERWISE be eligible as per KVS
admission guidelines.
iv)
The combinations of subjects are available in Kendriya
Vidyalaya.
v)
The admissions can be considered by the Assistant
Commissioner of the Region concerned subject to fulfilling of all the
conditions.

Admission for class I & Classes VI onwards :

single girl child may apply along with a copy of an affidavit stating that they
have single girl child and that there is no other sibling.

The state government school employees should submit the
service certificate signed by C.I. of School Sambalpur.
b)
The employees working in semi-govt./aided
college should submit the copy of the sanctioned order of the post by the competent authority
of higher education.
c)
The employees who are themselves head of the
institution will get the service certificate signed by the reporting
officer/next higher authority of the department.
d)
Those belonging to SEBC category will submit a copy of
certificate from competent authority (not below the rank of Tahsildar) for
school record.
